Barraquer, Barcelona"},"content":{"rendered":"<div class=\"rwd-video\"><iframe loading=\"lazy\" width=\"560\" height=\"315\" src=\"https:\/\/player.vimeo.com\/video\/701589952?autoplay=1?api=1&#038;player_id=vimeo_video&#038;color=#00adef\" allowfullscreen><\/iframe><\/div>\n<p>A unique case of a <em>Tenia solium<\/em> flat worm larva (<em>Cisticercus cellulosae<\/em>) that found its way into the vitreous chamber and appeared as a \u2018cataract\u2019. Edited by Joaqu\u00edn Barraquer in 1962. The initial sequence shows the cyst floating right behind the clear crystalline lens, and its changing appearance as the eye is moved with a forceps or with a Goldmann 3-mirror lens.<\/p>\n<p>The surgery involves a fornix-based conjunctival flap and placement of a Flieringa ring with 8 interrupted silk sutures. After a keratome incision, the aqueous humor is collected for laboratory studies, and a 200\u00ba incision completed with the J.I. Barraquer straight corneoscleral scissors. A corneoscleral suture is preplaced, and the posterior chamber is thoroughly irrigated with the aCT enzyme using an olive-tipped canula. A PI is performed with BDW scissors, colibri forceps and the help of a brush. Two minutes later, the chamber is washed with saline and the clear lens of this young patient is removed <em>in toto<\/em> with the erisophake -which required three attempts using increasing vacuum. The hyaloid face remains intact and the cyst is visible just behind it. The cornea is held open by the assistant with a traction suture as the surgeon compresses the globe with two instruments at the equator. The cisticercus cyst bulges with the intact vitreous but is not expressed out. Finally, the external compression maneuver is repeated -with extra hands- while the surgeon uses the marten-hair brush to gently massage the hyaloid until a hole opens in it, just over the intact cyst and through which it is then delivered, applying gentle pressure with another spoon. Noteworthy, the vitreous remains behind completely as the pupil is constricted with Ach irrigation, round and central. The preplaced suture is closed, followed by additional corneoscleral and conjunctival ones. At the end, the extracted larva can be seen -alive and moving its \u2018head\u2019 or scolex- in a Petri dish with saline.<\/p>\n<p>Un caso unico di larva di verme piatto di Tenia solium (Cisticercus cellulosae) che si \u00e8 fatta strada nella camera vitrea ed \u00e8 apparsa come una &#8220;cataratta&#8221;. A cura di Joaqu\u00edn Barraquer nel 1962. La sequenza iniziale mostra la ciste che fluttua proprio dietro il cristallino trasparente e il suo aspetto mutevole quando l&#8217;occhio viene spostato con una pinza o con una lente a 3 specchi Goldmann.<\/p>\n<p>L&#8217;intervento prevede un lembo congiuntivale a base di fornice e il posizionamento di un anello Flieringa con 8 punti di sutura in seta interrotti. Dopo un&#8217;incisione cheratomica e un&#8217;incisione a 200\u00ba completata con forbici corneosclerali dritte di Barraquer l&#8217;umor acqueo viene raccolto per studi di laboratorio . Viene preposizionata una sutura corneosclerale e la camera posteriore viene accuratamente irrigata con l&#8217;enzima aCT utilizzando una cannula a punta di oliva. Una PI viene eseguita con forbici BDW, pinze per colibri e l&#8217;aiuto di un pennello. Due minuti dopo, la camera viene lavata con soluzione fisiologica e il cristallino trasparente di questo giovane paziente viene rimosso in toto con l&#8217;erisofago, che ha richiesto tre tentativi usando un vuoto crescente. La faccia anteriore della ialoide rimane intatta e la cisti \u00e8 visibile appena dietro di essa. La cornea viene tenuta aperta dall&#8217;assistente con una sutura di trazione mentre il chirurgo comprime il globo con due strumenti all&#8217;equatore. La cisti del cisticerco si gonfia con il vitreo intatto ma non viene espressa. Infine, la manovra di compressione esterna viene ripetuta -in pi\u00f9 con le mani &#8211; mentre il chirurgo massaggia delicatamente la ialoide con la spazzola a pelo di martora fino a quando non si apre un foro in essa, appena sopra la cisti intatta e attraverso il quale viene poi erogata, esercitando una leggera pressione con un altro cucchiaio. Degno di nota, il vitreo rimane completamente dietro poich\u00e9 la pupilla \u00e8 ristretta con l&#8217;irrigazione di Ach, e rimane rotonda e centrale. La sutura preposizionata viene chiusa, seguita da ulteriori \u00a0punti corneosclerali e congiuntivali.
