It highlights the advantages of preplacing a suture, which is closed immediately after the lens extraction, and the uses of the marten-hair brush -gentler to soft tissues as the iris and less adherent than today\u2019s cell sponges-, spatulas, AC air bubble, and acetylcholine (Ach) irrigation to close the pupil.<\/p>\n<p>Case 1. Post-uveitis cataract requiring liberation of posterior synechiae with a flat spatula, and a sector iridectomy in 1 step, with moderate bleeding that stops spontaneously with irrigation. The very soft eye is shown by the concavity of the cornea after the lens extraction. After closing the preplaced suture, the position of the iris and lack of vitreous presentation is checked with the brush and an air bubble in the AC, and later with the spatula after completing the sutures.<\/p>\n<p>Case 2. Bleeding from the incision enters the chamber after an otherwise uncomplicated phakoeresis, attaching to the iris surface. This is partially removed with irrigation (Ach) and stabilized with an air bubble.<\/p>\n<p>Cases 3, 4 &amp; 6. Some material appears to be attached to the lens as it is extracted. This may consist of zonular fibers, which tend to retract spontaneously after the preplaced suture is closed and the pupil constricted with pilocarpine drops and\/or intracameral Ach. In the second case, an air bubble is gently injected and -as it does not fill the chamber- spatula sweeps are performed to check for\/liberate possible vitreous strands -without any external vitrectomy. In case 6, the material retracts, and the pupil constricts with Ach becoming round -indicating these were zonular fiber remnants-. Finally, the surgeon removes some minute remnants at the pupil edge with von Mandack forceps.<\/p>\n<p>Case 5. An intumescent cataract that breaks during open-sky extraction. There was increased zonular resistance (despite EZ?) and a relatively narrow pupil. The lens remnants are gently removed with the brush, and the pupil further constricted with Ach irrigation. As it stays round and central, no vitreous loss has occurred.<\/p>\n<p>Una serie di casi di cataratta oprati da Joaqu\u00edn Barraquer nel 1960, che mostrano la gestione di alcune complicanze durante l&#8217;estrazione intracapsulare con erisofago (facoeresi) assistita da EZ. Evidenzia i vantaggi della preposizionamento di una sutura, che viene chiusa subito dopo l&#8217;estrazione della lente, e l&#8217;uso della spazzola a pelo di martora -pi\u00f9 delicata sui tessuti molli come l&#8217;iride e meno aderente delle odierne spugne cellulari-, spatole, bolle d&#8217;aria AC, e irrigazione con acetilcolina (Ach) per chiudere la pupilla.<\/p>\n<p>Caso 1. Cataratta post-uveite che richiede la liberazione delle sinechie posteriori con una spatola piatta e un&#8217;iridectomia di settore in 1 passaggio, con sanguinamento moderato che si interrompe spontaneamente con l&#8217;irrigazione. L&#8217;occhio molto morbido \u00e8 mostrato dalla concavit\u00e0 della cornea dopo l&#8217;estrazione della lente. Dopo aver chiuso la sutura preposizionata, la posizione dell&#8217;iride e la mancanza di presentazione vitrea viene controllata con il pennello e una bolla d&#8217;aria nell&#8217;AC, e successivamente con la spatola dopo aver completato le suture.<\/p>\n<p>Caso 2. Il sanguinamento proveniente dall&#8217;incisione entra nella camera anteriore dopo una facoeresi altrimenti semplice, attaccandosi alla superficie dell&#8217;iride. Questo viene parzialmente rimosso con l&#8217;irrigazione (Ach) e stabilizzato con una bolla d&#8217;aria.<\/p>\n<p>Casi 3, 4 e 6. Sembra che del materiale sia attaccato alla lente mentre viene estratto. Questo pu\u00f2 essere costituito da fibre zonulari, che tendono a ritrarsi spontaneamente dopo che la sutura preposizionata \u00e8 stata chiusa e la pupilla ristretta con gocce di pilocarpina e\/o Ach intracamerale. Nel secondo caso, viene iniettata delicatamente una bolla d&#8217;aria e, poich\u00e9 non riempie la camera, vengono eseguite\u00a0 con spatola manovre per verificare\/liberare eventuali filamenti vitreali senza alcuna vitrectomia esterna. Nel caso 6, il materiale si ritrae e la pupilla si restringe con Ach che diventa rotonda, indicando che si trattava di resti di fibre zonulari. Infine, il chirurgo rimuove alcuni minuscoli resti sul bordo della pupilla con una pinza von Mandack.<\/p>\n<p>Caso 5. Una cataratta intumescente che si rompe durante l&#8217;estrazione a cielo aperto. C&#8217;era una maggiore resistenza zonulare (nonostante EZ?) e \u00a0una pupilla relativamente stretta. I resti della lente vengono rimossi delicatamente con il pennello e la pupilla viene ulteriormente ristretta con l&#8217;irrigazione Ach.
